For me it's a toss up between YOLT Blofeld's eye scar and Emilio Largo's eye patch. Ultimately, I went with YOLT Blofeld's eye scar.

I see that Kristatos' "Thing for underage girls?" is still out in front. That's just so Bond And Beyond.
As for Sanchez, not quite sure what else I could have put. Iguana? Big f*ck-off Stinger missiles? President in back pocket? Whips his women? The coke empire just seems like his main big thing.

#1: Dr. No.
The original exotic deformity, which he used to his advantage until they led to his demise.
Everything after was an imitation.
#2: Jaws (in TSWLM) - the best of the imitations (in MR he was more of an irritation).
#3: Scaramanga. True, his 'superfluous papila' was little more than a source for several jokes, but more was made of it in the film that in the book.
#4: Renard. A man who cannot feel pain and doesn't care about anything because he's dying anyway? I wouldn't want to meet such an adversary.
#5: Zao, just for being so damn creepy and exotic-looking (at least we got to see him before, as well as after).

I'm most partial to Oddjob's metal brimmed bowler. That's a classic.
Honorable mention to Klebb's spiked shoes, Grant's garotte-friendly wrist-watch, and Blofeld's eye scar.
I would appear to stuck in the 1960's. Not a bad place to be though when we're talking 007.
